I think that somehow my /etc/printcap or apsfilter installation was corrupted. I deleted my /usr/local/lib/apsfilter directory, deleted the printer subdirectories under /var/spool/lpd, and deleted the /etc/printcap file. Then I tar -xvzf the apsfilter distribution again into /usr/local/lib, edited the global/GLOBALS.sh file to correct the path for TEX, and ran the SETUP again with all the same settings. This time it got it right and KDE2 and Netscape print fine now. I'm not sure what the actual problem was but in KDE2 print dialog, it now shows the "Alias" comments for both lp and raw correctly.
