Are you sure that's coming from apsfilter?  I don't believe it is.
Remember, apsfilter is an ASCII to PostScript filter, and nothing more.
I think you should be looking at things like your printing system
configuration (lpd or LPRng or CUPS or whatever).  That's usually what
generates such things, since those entities have visibility over and/or
control things like what user, what queue, what time, and so forth.

I've done grep -ir on all doc-type files in the apsfilter distribution, and
on all files in the installed version, in search of 'cover' and 'banner',
and find absolutely no reference in any way.  If it could be 'turned off',
something would have been textually referencing that functionality.
