FWIW, I too am using Slackware 8.1. As a *non-root* user, I am printing
to an HP-656c which is on a windows 98 system.

Before running SETUP I did 'mv /etc/printcap /etc/printcap.0' because I
wanted to ensure that I had 'lp' as my default. Otherwise, this was a
clean/pristine Slackware 8.1 install. SETUP wrote a new etc/printcap.
Be sure to select 'o'verwite, not 'a'dd in SETUP.

I have seen the stair-step print when printing from gimp. This was
solved, in gimp ,by using the print setup button and choosing the HP
Deskjet 600/600C instead of Postscript level 2. That this selection
works is interesting because my 656 is in the AP2000 series, whereas
your 612 is in the 600 series. Look in
/usr/doc/hpijs-1.1/hpjs-readme.html. Your 612 s/b ok.

Printing from mozilla works as default.

echo "test"|lp works.
